    How You’ll Make an Impact

    • Observes and documents any patient-reported medication-related concerns, such as   side effects, allergies, or changes in functional status, and communicates these to the supervising OT.
    • Advises and consults with clients and their families regarding strategies to support the client’s progress toward achieving their highest level of functional potential.
    • Observes, documents, and maintains accurate clinical records of client activities and responses to treatment; reports significant changes in condition or reactions to the physician in a timely manner.
    • Communicate effectively and professionally with all members of the care team to ensure coordinated and quality client care.
    • Guides and instructs the client in prescribed therapeutic self-care and creative activities that are directed toward improving independence and physical and mental functioning.
    • Provides instruction to clients in the safe use and care of wheelchairs, braces, splints, and prosthetic and orthotic devices.
    • Assists in teaching exercise programs to patients and their families.
    • Performs all other duties as assigned.

    QUALIFICATIONS EDUCATION and/or EXPERIENCE

    To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

    • Associate’s degree (A.A.) or equivalent from an accredited two-year college or technical school; or six months to one year of related experience and/or training; or an equivalent combination of education and experience.

    • Minimum of one (1) year of clinical experience preferred; new graduates may be considered with strong clinical rotations in home health or geriatrics.

    • Proficiency in electronic medical record (EMR) systems, mobile documentation, and basic telehealth platforms.

    • Reliable transportation required for home health assignments.

    • Ability to communicate effectively in English, both verbally and in writing.

    • Demonstrated ability to work collaboratively within a team as well as independently.

    • Strong organizational skills with the flexibility to adapt to changing priorities.

    • Ability to adapt to varied home environments while maintaining safety standards.

    • Proficient computer skills.

    CERTIFICATES, LICENSES, REGISTRATIONS

    •    Valid State of Florida Certified Occupational Therapy Assistant (COTA) Licensure

    •    Valid State of Florida Driver’s License 

    •    Valid BLS CPR Certification
