Virtual support assistant role to support front line clinical staff in care of patients receiving palliative care in the home setting. Serve as an extension to front line staff in patient care logistics management including patient outreach, consent, scheduling. Responsible for coordinating with clinical staff to ensure scheduling efficacy, provider orders carried out, delivered, and vendor interfaces successful for order completion. Support care of home based census of patients.
