The Role
We're looking for<!----> a Bilingual (Spanish/English) Care Coordinator to be the connective thread between patients, families, clinicians, and physicians. In this role, you will play a crucial part in supporting our Spanish-speaking patients and families to ensure seamless communication and culturally competent care. In addition to remote case management, you will travel on-site to work directly with clients and team members for in-person evaluations, assessments, and ongoing training.
What You'll Do
- Travel on-site to clients and employees to complete various in-person assessments, evaluations, and trainings.

- Track authorizations, re-certifications, and service frequency requirements.<!----><!----><!----><!----> <!----><!----><!----><!----><!----><!---->
- Communicate with physicians and clinicians regarding changes to the plan of care.<!----><!----><!----><!---->
- Monitor case status and flag gaps in service or documentation.<!----><!----><!----><!----> <!----><!----><!----><!----><!----><!---->
- Maintain accurate re<!---->cords in company systems, including HHAeXchange and Salesforce.<!----><!----><!---->

What You Bring
- Fluency in Spanish and English (spoken and written) is required.
- Experience in home care, healthcare coordination, or case management preferred.
- Strong organizational skills and comfort managing multiple cases at once.
- Clear, professional communication with patients, families, and clinicians.
- Proficiency with Google Workspace and care management systems.
- Ability to work independently in a hybrid, remote-first environment.
