    Job description

At New Concepts for Living (NCFL), we believe great care starts with seeing the whole person—not just the diagnosis. Our team works every day to help adults with developmental disabilities build skills, reach personal goals, and experience greater independence. We’re looking for a Physical Therapist (PT) who is ready to bring clinical expertise, leadership, and compassion to a role where your decisions can have a lasting impact.

    • Provide habilitative and rehabilitative physical therapy services to adults with I/DD and individuals with physical, cognitive, and/or psychological needs.

    • Develop individualized treatment plans aligned with each individual's Individualized Service Plan (ISP).
    • Adapt and modify treatment plans and interventions as clinically indicated.
    • Supervise and provide clinical direction to Physical Therapist Assistant(s) (PTA).
    • Complete recertifications, progress notes, discharge summaries, and other required documentation accurately and on time.
    • Communicate with Support Coordinators to promote implementation of therapy recommendations in home and community settings.
    • Educate direct support staff on recommendations, exercises, and strategies to be implemented outside of therapy sessions.
    • Collaborate with healthcare providers and members of the individual's care team to promote effective, coordinated care.
    • Participate in multidisciplinary team meetings and contribute clinical expertise to support positive outcomes.
    • Maintain accurate records of evaluations, progress, treatment plans, and other clinical documentation in compliance with legal, ethical, and agency standards.
    • Follow directives from the Therapy Director and adhere to NJ DDD guidelines for habilitative and rehabilitative therapy services.

    Why You’ll Love Working Here:

At NCFL, your expertise has a direct impact on the people we support. You’ll have the opportunity to lead clinically, collaborate across disciplines, and see your recommendations translate into meaningful progress in the lives of the individuals we serve.

    • Company-paid life insurance
    • Paid Time Off (PTO)
