    Speech Language Pathologist (SLP) PRN Home Health

    We are seeking a PRN Speech Language Pathologist to serve patients in the Middlebury and Danbury area. You will have the opportunity to provide personalized care while working with a collaborative and supportive team.

    What You’ll Do

    • Evaluate patients and develop individualized plans of care.
    • Provide speech and language therapy services according to physician orders.
    • Help patients improve their communication, cognitive, and swallowing abilities.
    • Educate patients and caregivers on home therapy programs.
    • Maintain accurate and timely clinical documentation.
    • Follow OASIS, PPS, and company guidelines.
    • Collaborate with physicians and interdisciplinary team members to ensure continuity of care.

    What We’re Looking For

    • Master’s degree in Speech Language Pathology
    • Current and unrestricted SLP license in the state of practice
    • At least one year of clinical experience as a Speech Language Pathologist
    • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
    • Ability to travel throughout the Middlebury and Danbury coverage area
    • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment

    The base salary for this position is $70.00 to $80.00 per point, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
