The Transitional Housing Support Worker provides trauma-informed, resident-centred support to women and families residing within the transitional housing program. 

Support Workers are responsible for supporting residents in achieving housing stability, safety, independence, and overall wellbeing while contributing to a safe, supportive, and welcoming residential environment. 

The role combines resident support, housing stabilization, crisis intervention, family support, house operations, documentation, advocacy, and community resource navigation.

What you’ll do:

Resident Support & Advocacy

• Apply a trauma-informed and survivor-centered approach, prioritizing the safety and well-being of the clients. 
• Build positive, respectful, and professional relationships with residents. 
• Conduct resident check-ins and wellness assessments. 
• Provide emotional support, encouragement, coaching, and practical assistance. 
• Support residents in identifying, developing, and achieving personal, family, and housing-related goals. 
• Advocate for residents and assist them in navigating service systems. 
• Support resident engagement in programming, community activities, and life-skills development. 
• Facilitate resident meetings, workshops, groups, and recreational activities as required. 

Intake, Orientation & Housing Stabilization 

• Conduct resident intakes, assessments, and orientations. 

• Support resident admissions, move-ins, room transfers, and move-outs. 
• Assist residents in securing and maintaining housing. 
• Support residents in completing applications, referrals, and service documentation. 
• Connect residents with housing, employment, educational, legal, health, financial, and community resources. 
• Assist residents in overcoming barriers to independence and stability. 
• Support residents in navigating community systems and accessing available services. 

Crisis Intervention, Safety & Risk Management

• Respond appropriately to crises, emergencies, conflicts, and resident concerns. 
• Utilize trauma-informed and de-escalation practices. 
• Assist residents in developing and implementing safety plans. 
• Conduct risk assessments and identify emerging concerns. 
• Monitor resident wellbeing and building safety. 
• Contact emergency services when required. 
• Escalate significant concerns in accordance with organizational policies and procedures. 
• Complete incident reporting and follow-up documentation. 

Family & Child Support 

• Promote family wellbeing and healthy family functioning. 
• Support women, children, and families residing in the program. 

• Collaborate with the Child & Youth Worker and Mental Health Counsellor as appropriate. 
• Assist families in accessing parenting, child, youth, and family support services. 
• Encourage participation in child and family programming and activities. 

House Operations & Program Support

• Contribute to the creation and maintenance of a safe, clean, and welcoming residential environment. 
• Conduct routine health, safety, and security checks. 
• Monitor common areas and shared spaces. 
• Report maintenance, safety, and facility concerns. 
• Assist with inventory management and distribution of supplies. 
• Support donation management and organization. 
• Assist with transportation coordination, appointments, and operational logistics as required. 
• Support the day-to-day operations of the program and residence. 

Documentation, Communication & Reporting

• Maintain accurate and timely resident records. 
• Complete shift notes, case notes, communication logs, and reports. 
• Document resident interactions, referrals, progress, concerns, and follow-up actions. 
• Complete incident reports and required program documentation. 
• Participate in shift handovers and communication processes. 

• Ensure documentation is objective, professional, and completed within required timelines. 
• Maintain confidentiality in accordance with organizational policies and legislation. 
• Track and compile monthly KPI reports.   

Team Collaboration & Professional Development

• Participate in mandatory bi-weekly team meetings. 
• Engage in ongoing training and professional development. 
• Collaborate with fellow Support Workers, the Home Manager, Child & Youth Worker, Mental Health Counsellor, volunteers, and community partners. 
• Contribute to a positive team culture and shared accountability model. 
• Support continuity of care by ensuring effective communication across shifts.

What we're looking for

• Degree or Diploma in a related human/social service field such as Social work or Social service work OR Child and Youth Care Work and one (1) year of recent related experience. Or an equivalent combination of relevant training, education, and experience.
• Social perception to work with individuals who have difficulty expressing their needs.
• Demonstrated skills in case management, assessment, and intervention, writing case notes, advocacy, conflict resolution techniques, mediation, de-escalation skills and non-violent crisis management.
• Knowledge of crisis-intervention methodologies.
• Ability to compromise, negotiate, and work well with others.
• Current First Aid/CPR Certificate is an asset.
• Access to a reliable vehicle is an asset.
• Successful candidates must provide a satisfactory Vulnerable Sector Check prior to commencing employment.
