Port Hawkesbury Health Centre is a collaborative, patient-centered practice and a strong example of a Health Home model of care. The interdisciplinary team includes four physicians, a nurse practitioner, family practice nurse, dietitian, social worker, licensed practical nurse, continuing care coordinator, and five dedicated administrative staff who work together to provide comprehensive, coordinated care. The clinic has recently expanded to include additional exam rooms, along with an enhanced phone and patient appointment reminder system that supports efficient workflows and improved access for patients. This team-based environment allows providers to focus on high-quality, holistic care while being well supported in their day-to-day practice.
