Position Overview:
The Transformation Consultant will function as a technology implementation coach, providing health center clinicians and leaders with strategies and tools to strengthen and improve operational excellence and health outcomes. This position provides comprehensive technical assistance to optimize clinical practice through leveraging EHR functionality and data to improve quality performance and patient outcomes. The position works to define and redesign clinical and operational workflows, partnering directly with clinical end users to help map workflows, identify barriers to performance, and connect data to real-world behavior (e.g., how missed visits affect quality and cost). The position applies a strong understanding of primary care operations, including patient flow, scheduling, provider workflows, and care team roles, to assess performance drivers, and can help identify gaps related to workflow issues, documentation, and/or data/reporting challenges. Please note: This position is funded through a contract. Continued employment is contingent upon contract renewal or extension, and organizational needs.
