Join our Hospice House/Inpatient Unit team as a Registered Nurse and provide compassionate, patient-centered care to individuals with life-limiting illnesses. You’ll support patients and their families throughout their hospice journey while working closely with an interdisciplinary team in a collaborative environment focused on comfort, dignity, and clinical excellence.
Provide skilled nursing care using the nursing process, including comprehensive assessment, planning, implementation, and evaluation.
Assess and address patients’ physical, emotional, psychosocial, and spiritual needs while promoting comfort, dignity, and quality of life.
Develop, implement, and regularly update individualized plans of care in collaboration with patients, families, and interdisciplinary care teams.
Monitor patient conditions, recognize changes in status, and promptly communicate significant findings to physicians and other healthcare providers.
Evaluate and modify nursing interventions, medications, and care plans based on changes in patient condition and clinical needs.
Participate in patient admissions, interdisciplinary team meetings, care conferences, and ongoing care coordination.
Maintain accurate, timely, and thorough documentation of assessments, interventions, physician orders, medications, and patient status in the electronic medical record (EMR).
Provide clear and effective patient handoffs using SBAR and other standardized communication techniques to ensure continuity and safety of care.
Provide guidance, supervision, and support to personal care specialists, volunteers, and other caregivers to ensure care is delivered according to established plans and standards.
Collaborate effectively with physicians, nurses, support staff, and other departments to promote high-quality, patient-centered care.
Participate in quality improvement initiatives focused on patient safety, outcomes, regulatory compliance, and organizational excellence.
Serve as a preceptor and mentor for new team members, providing clinical guidance and support during orientation and training.
