Position Title: Survivor Care Coordinator II

Employment Classification: Full Time / Hourly / Non-Exempt

Shift: Sunday-Thursday 9am-5:30pm

Reports to: Housing Supervisor

Department: Housing

Position Summary:

The Survivor Care Coordinator is responsible for providing direct participant-led support and coordination of care for current residents of The Lighthouse/Graduate Apartments, and prospective young adults seeking to move into survivor-centered housing, which may include other transitional housing sites. This position requires a flexible schedule that includes weekends.

Major Areas of Responsibility: 

• Provide direct support and care to youth using a trauma-responsive, survivor centered and harm reduction approach as a core philosophy.
• Work from a survivor centered approach, empowering residents to explore their individual needs and strengths and develop case plans based on resident’s self-identified goals.
• Plan, coordinate and motivate youth to engage in support services based on youth’s needs, to support the successful obtainment of goals.
• Implement Stages of Change frameworks to support residents to maintain a safe, survivor led, home environment, including feedback from residents to inform the programming and guidelines of the program.
• Support residents to develop an awareness and understanding of individual safety and continually safety plan using a harm reduction approach.
• Provide crisis intervention and conflict resolution as needed, using therapeutic interventions that encourage and empower residents to develop and use their own communication skills to discuss the issue causing the conflict.
• Work collaboratively with youth identified support, communicating about shared clients to meet the needs of youth receiving services from all programs.
• Review program applications and participate in the intake process, participation in interviews with potential new residents and assist with decisions regarding a youth’s readiness for the program.
• Coordinate the youth’s move in process, logistics of their move-in and ensure the emotional needs of youth are met.
• Support the youth’s move out process and connect youth to aftercare services to help youth sustain the skills needed to maintain their housing.
• Provide collaboration and advocacy with other service providers on behalf of residents and attend case conferences and court as needed.
• Maintain confidentiality related to program participation.
• Additional duties as assigned by Program Leadership.
