Position: Licensed Practical Nurse (Part-time)
The Licensed Practical Nurse performs various aspects of the client medical care under the supervision of a Registered Nurse. The LPN directly provides and/or coordinates health services and health-related activities for clients participating in or completing treatment.
Essential Duties:
- Administer medication, including intermuscular injection, as ordered by MD.
- Conduct substance use assessments to determine appropriateness for participation in treatment, need for physical exam or other medical services.
- Follow up on any health related referrals made.
- Maintain a tracking system to monitor patient health issues, medical appointments, and prescribed medications (internal/external).
- Provide patient education and counseling around health related issues.
- Provide staff education around health related issues.
- Provide crisis intervention on an emergency basis when indicated.
- Complete all required documentation and reports in a timely manner.
- Actively participate in staff meetings, case conferences, and other unit meetings.
- Maintain communication and teamwork with clinical staff to integrate and enhance clinical services.
- Conduct toxicology testing as needed.
- Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
- Ensure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan and personnel policy and procedure.
- Identify the current health needs of the patient population and implement activities to address service and informational deficits.
- NYS Licensed Practical Nurse (LPN).
- Previous experience working in an Opioid Treatment Program/Substance use treatment program preferred.
- Bilingual English/Spanish a plus.
- Experience with AVATAR electronic health records a plus.
