About the Role
Role & Team Overview (Shift Caregiver – Adult Foster Home)
As a Shift Caregiver in our Adult Foster Home (AFH), you will provide direct, hands-on support to residents during assigned scheduled shifts in a small, home-like setting. This role is responsible for maintaining consistent shift coverage, following the daily flow of the home, receiving and giving clear handoffs, and carrying out resident care duties throughout the shift. You will support residents with daily routines, safety, comfort, supervision, and quality of life while following each resident’s care plan, house procedures, and delegated responsibilities. Strong communication, reliable attendance, teamwork with your shift partner, timely documentation, and prompt reporting of changes or concerns are essential to keeping residents safe and ensuring smooth continuity from one shift to the next.
The Team You’ll Be Joining
We are a Specialized Contract Home and maintain a consistent staffing model of two staff to five residents at all times. As a Shift Caregiver, you will work scheduled shifts side-by-side with another caregiver, ensuring continuous coverage and the ability to respond quickly to resident needs. We place a strong emphasis on teamwork and a positive, respectful work environment—staff coordinate throughout the shift, share updates in real time, step in to help when a teammate is busy, and approach challenges with a “we’ve got this together” mindset. Our goal is a calm, supportive home for residents and a supportive team culture where everyone feels heard, backed up, and set up to succeed.

What You’ll Do
How the Role Works Day-to-Day
- Work assigned shifts reliably: Arrive on time, follow care plans, house routines, and shift instructions, and communicate promptly if coverage may be affected.
- Provide team-based care: Receive handoff, review resident updates, and work closely with your shift partner to share tasks, assist with transfers, support breaks, and respond to resident needs.
- Support resident daily care: Assist with ADLs such as bathing, dressing, grooming, toileting, mobility, meals, hydration, comfort routines, and other care needs based on each resident’s plan.
- Maintain safety and wellness: Monitor residents for changes in condition, behavior, appetite, mobility, pain, skin condition, sleep, or safety risks, and follow fall prevention, infection control, emergency, and house safety procedures.
- Assist with medications, as authorized: Complete delegated medication tasks according to policy, including medication pass, MAR documentation, med box support, reporting concerns, and tracking refills.
- Document and report: Record care provided, observations, incidents, refusals, changes, and medication-related information, and report concerns to the appropriate lead, supervisor, nurse, or medical contact.
- Complete shift handoff: Share clear updates with incoming staff, finish assigned duties, documentation, cleaning or restocking tasks, and leave the home organized for the next shift.
Qualifications
- Must be at least 18 years of age.
- Must be able to pass a background check and any other screening required for work in an Adult Foster Home.
- CPR training/certification required, or ability to obtain within a specified timeframe.
- EQC training and certification required. If not currently certified, applicant must have at least three years of experience supporting individuals with Level 3 care needs and be willing to complete the required courses and training to become properly certified. Course costs are the employee’s personal expense; however, with a commitment, Pacific Gateway may offer reimbursement options.
