Position Summary:
Active member of the Targeted Case Management Unit within the Philadelphia Office of Mental Health. Provides Adult and Children Targeted Case Management (TCM) Units located in various Community Mental Health Centers and Specialized Agencies with technical assistance and support pertaining to identified problems and issues that may need to be addressed on an individual, agency or systemwide basis. Involved with a variety of “specialized" projects that pertain directly /indirectly to TCM service development across the developing Behavioral Health system. Performs liaison role to other City and/or State departments (i.e > IDS/ODP, OSH, OMHSAS, Prisons/Courts, DHS, etc) as it pertains to TCM initiatives and functions.
Duties and Responsibilities:
Skills Required:
Working knowledge of the Philadelphia TCM system as well as the developing Behavioral Health Care components. Direct experience as a case manager and/or supervisor a major plus. Well versed in the DBHIDS Practice Guidelines, Philadelphia Mental Health system CSP/CAASP components - locations, types, accessibility, criteria, etc. A clear understanding of the TCM rules and regulations (both a county and state perspective/expectation) for service provision from the provider agencies
Ability to develop and execute trainings/technical assistance learning opportunities relevant/relative to case management (adult/child) initiatives.
Clinical, assessment/evaluation skills particularly as it relates to TCM are required.
Flexible, personable style, with strong verbal and written communication skills is recommended. Knowledge of computers (Microsoft Office applications) a plus.
Education and Experience:
Individuals with mental health lived experience are encouraged to apply. Minimum of a Bachelors Degree in the Health Care field and five (5) years direct experience with TCM in the Philadelphia Mental Health System. Masters degree preferred.
