Job Title: Home Health Aide (HHA) / Certified Nursing Assistant (CNA)

Position Summary:
The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) provides compassionate, client-centered personal care and supportive services to clients in their homes. Working under the supervision of the Nursing Supervisor and the direction of the Scheduling Coordinator, the HHA/CNA helps maintain the client’s comfort, safety, and dignity while promoting independence.
Key Responsibilities:
Client Care:
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, ambulation, and feeding.
- Provide assistance with prescribed exercises and mobility (as trained and permitted by regulation).
- Provide medication reminders as directed by nursing supervisor.
- Take and record vital signs, observe and report changes in client condition to the Nursing Supervisor as directed.
- Prepare meals and assist with feeding as needed.
- Perform light housekeeping to support a safe and clean home environment.
- Provide companionship and emotional support.
Documentation & Communication:
- Accurately document care provided and report changes in condition or concerns promptly.
- Use GPS/EVV app and company software for clocking in and out of shifts.
- Communicate regularly with the Scheduling Coordinator regarding assignments, availability, and scheduling needs.
- Follow the individualized care plan as directed by nursing staff.
Supportive Services:
- Provide companionship, socialization, and emotional support to clients.
- Assist with meal planning and preparation according to dietary guidelines.
- Perform light housekeeping duties such as laundry, dishwashing, vacuuming, dusting, and general tidying.
- Assist with errands, grocery shopping, and escort to appointments or social activities (as applicable).
- Provide reminders for medications(without administering), appointments, and hydration.
Client Safety & Communication:
- Observe and report changes in the client’s condition or home environment to the Nursing Supervisor.
- Communicate effectively with the Scheduling Coordinator regarding schedule changes, availability, and client concerns.
- Ensure the client’s living environment is safe and free from hazards.
Compliance & Conduct:
- Adhere to agency policies, procedures, and applicable state regulations.
- Maintain client confidentiality in accordance with HIPAA guidelines.
- Attend required training sessions and in-service education.
- Maintain professional boundaries and uphold the agency's code of conduct.
