The Patient Care Coordination Medical Assistant serves as a key member of the ambulatory care team, supporting primary care patients through centralized management of incoming calls, electronic messages, and care coordination activities. This position is responsible for addressing patient inquiries within Medical Assistant scope of practice, facilitating timely communication between patients and care teams, and ensuring accurate documentation in the electronic health record (EHR). The role promotes exceptional patient experiences, continuity of care, and efficient workflow management while ensuring that clinical concerns requiring assessment or triage are appropriately escalated to licensed clinical staff.
