Caregiver

QUALIFICATIONS:

1. Must have at least one year of certifiable experience in home care, private-duty home care setting, community health agency, hospital or med-surge clinical rotation in a nursing school or have at least one year of verifiable work experience in an unrelated field and if in an employed state where necessary, have successfully completed a Home Health Aide Training course approved by the Vice President of Home Care.

2. Demonstrate an ability to effectively communicate in English, and/or the predominant language of the population being served, and must be able to read and write.

3. Pass the Bedrock at Home Competency Evaluation Examination.

4. Demonstrates competency in performing all skills.

5. Pass a criminal background check.

6. Meet all medical requirements.

7. Meet all state requirements.

8. Have reliable transportation and a method of communication.

9. Is punctual, dependable, and demonstrates sound judgment.

JOB RESPONSIBILITIES:

1. Follows the established Plan of Care.

2. Assists with activities of daily living, which means the functions of tasks for self-care that are performed either independently or with supervision or assistance. Activities of daily living include mobility, transferring, walking, grooming, bathing, dressing, undressing, eating, and toileting, per the Plan of Care.

3. Assists with Independent Activities of Daily Living, which may include light housekeeping, shopping, laundry, and meal preparation, per the Plan of Care.

4. Accompany clients to appointments (transportation is the client's responsibility unless a driving waiver is signed and in the Plan of Care).

5. Provides assistance with self-administering, pre-drawn, and medication reminders, per Plan of Care.

6. Understands and maintains infection and safety measures.

7. Reports any and all changes in the client's condition to the Care Manager or BOM.

8. Completes the Activity Records and accurately documents the care provided.

9. Performs all activities as directed by the Care Manager or BOM

10. Has a positive relationship with clients and families.

11. Has positive client satisfaction.

12. Meets all deadlines for health screening requirements and training.

13. Encourage maximum independence to facilitate rehabilitation as determined by EP and POC.

14. Respect the client's rights and ensure confidentiality.
