We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our Wellness team. The LPN will provide hands-on resident care, administer medications, maintain accurate documentation, and work closely with the Registered Nurse and care team to support residents’ health, safety, and well-being.
Responsibilities:
- Provide resident care in accordance with individualized service plans.
- Assist residents with activities of daily living as needed.
- Administer prescribed medications and accurately document all services provided.
- Monitor residents for changes in health, behavior, or functional status and promptly report concerns to the Registered Nurse.
- Assist with developing and updating resident service plans and care documentation.
- Respond to resident emergencies, provide first aid, and arrange appropriate medical follow-up.
- Communicate with physicians and other healthcare providers as directed.
- Provide residents with compassionate one-on-one support and reassurance.
- Help maintain a safe, comfortable, and respectful environment for residents.
- Support the Registered Nurse with staff training, assignments, and other clinical responsibilities as needed.
- Follow all community policies, procedures, and applicable healthcare regulations.
- Promptly report resident incidents and work-related injuries.
Qualifications
- Current and active Licensed Practical Nurse (LPN) license in the State of New Jersey.
- Successful completion of an approved practical nursing program and the required licensing examination.
- Strong communication, organizational, and documentation skills.
- Compassionate, dependable, and committed to providing quality senior care.
- Prior experience in assisted living, senior living, or long-term care is preferred.
Shift:
- 7:00am-3:00pm
- 11:00pm-7:00am
- Weekends
