    DEPARTMENTAL SUMMARY The Rehabilitation Department provides therapy and nursing services to patients, families and patient’s caregivers including: Rehabilitation Nursing Care, Physical Therapy, Occupational Therapy and Speech Language Pathology. Our department operates as a system department and provides services to Acute Care, Inpatient Rehabilitation Facility and Outpatient settings across four campuses. Our Rehabilitation departments offer a variety of specialty services across the various services delivery areas.

    POSITION OVERVIEW: The Physical Therapist evaluates, plans, and provides treatment and rehabilitation as prescribed by a referring physician. Works with family and team members regarding patient’s abilities and goals.

    This position does not directly manage any other caregivers.

    ESSENTIAL FUNCTIONS AND DUTIES:

    Adheres to Oregon State Physical Therapy Licensing Board.

    Provides physical therapy evaluation and treatment to achieve desired outcomes.

    Provides staff consultation and training.

    Provides community resource knowledge and referral services.

    Provides physical therapy services to other hospital service areas.

    May be required to drive Rehabilitation van and meet all related SCHS vehicle policy requirements.

    Functions as a member of the Interdisciplinary Care Team to assure patient care is accomplished effectively and efficiently in a cost effective manner.

    Participates in clinical care conferencing.

    Gathers data and accurately documents information in a timely manner demonstrating compliance with departmental and professional regulations.

    Participates with team members in facilitating patient’s and family’s learning throughout continuum of care.  Reinforces patient’s continued health care through teaching and/or referral to community agency follow-up.

    Provides and maintains a safe environment for caregivers, patients and guests.

    May perform additional duties of similar complexity within the organization, as required or assigned.

    EDUCATION:

    Required:  Bachelors, Masters, or Doctorate degree in Physical Therapy from a college or university accredited by the American Physical Therapy Association.

    LICENSURE/CERTIFICATION/REGISTRATION:

    Required:  Must have necessary requisites for licensure as a Physical Therapist by the Oregon State Board of Health. Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements. Ability to travel to business functions/trainings/meetings and all St. Charles Health System worksites. Current AHA Basic Life Support for Healthcare Provider certification.

    Preferred:  Preferably a member in good standing of the American Physical Therapy Association. See additional position information for preferred certification list.

    EXPERIENCE:

    Required: Must have one year experience with therapy procedures, current orthopedic and neurological follow-up procedures, and rehabilitation practices.

    Preferred: Previous physical therapy experience in hospital-based setting.

    PERSONAL PROTECTIVE EQUIPMENT:

    Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

    Hand Therapy Certification Commission: Certified Hand Therapist (CHT).

    NAIOMT: Certified Orthopedic Musculoskeletal Therapist (COMP/CMPT).

    Rehabilitation Engineering and Assistive Technology Society of North America: Assistive Technology Professional Certification (ATP). Seating and Mobility Specialist (SMS).

    International Loving Touch Foundation: Certified Infant Massage Instructor Certification (CIMI).

    LSVT Global: LSVT BIG.

    Evergreen Certifications: Certified Clinical Anxiety Treatment Professional (CCATP). ADHD-Certified Rehabilitation Services Provider (ADHD-RSP), Certified Stroke Rehabilitation Professional (CSRP).

    Physiological Oncology Rehab Institute: PORi Certification for oncology therapy (PORi)

    Neurorecovery Unlimited: Certified Stroke Rehabilitation Specialist (CSRS)

    Neonatal Therapy Certification Board: Neonatal Therapy Certification (NTCB)

    Communication/Interpersonal

    Basic to intermediate ability and experience in computer applications, specifically electronic medical records system and MS Office.

    PHYSICAL REQUIREMENTS:

    Continually (75% or more): Standing and walking, keyboard operation, use of clear and audible speaking voice and the ability to hear normal speech level. 

    Frequently (50%): Sitting, lifting/carrying/pushing or pulling 1-25 pounds. The use and operation of a motor vehicle for Home Health and Wound Caregivers.

    Occasionally (25%): Bending, stooping/kneeling/crouching, climbing ladder/step-stool (varies by area), reaching overhead, lifting/carrying/pushing or pulling 25-50 pounds, grasping/squeezing, ability to hear whispered speech level.

    Rarely (10%): Climbing stairs.

    Never (0%):  Climbing ladder/step-stool (varies by area), operation of a motor vehicle.

    Exposure to Elemental Factors

    Rarely (10%): Wet/slippery area, chemical solution.

    Never (0%):  Heat, cold, noise, dust, vibration, uneven surface.
