Transition Advisor is a part-time (5-10 hours/week) hourly role earning $19/hour
What you will do:
- Manage caseload with required one-to-one sessions
- Implement and modify curriculum for classes
- Facilitate and/or Co-Facilitate classes based on setting goals, navigating resources, and increasing independence
- Provide personalized support that is aligned with programmatic goals and outcomes
- Complete monthly evaluations for each individual on caseload
- Maintain and update activities and resources focused on transition-aged individuals

What we expect of you:

- Bachelors degree in social work, special education, non-profit management, or related field preferred
- Two or more years of experience working with transition-age individuals with intellectual and/or developmental disabilities preferred

- Willingness and ability to learn various technology platforms and effectively teach participants how to use them
- General knowledge of special education services and post-high school options
- Strong understanding of effective ways to support teens and young adults with disabilities
- Strong relationship-building skills with individuals, families, and colleagues
- Experience leading and guiding teens and young adults in both individual and group settings
- Confidence facilitating curriculum for both small and large groups
- Resourcefulness in navigating technology, applications, and community-based resources
