Suburban Home Health is a jointly owned venture between several Suburban Health Organization hospital systems here in central Indiana! - Provides personal care activities of daily living such as bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing.
- Communicates changes in patient status to the supervising Registered Nurse (RN) in a timely manner.
- Reports patient complaints to the RN
- Creates and submits legible, accurate and timely documentation.
- Performs duties in compliance with agency policies and procedures and local, state, and federal regulations.
- Demonstrates attention to details for patient's safety and safety of their home environment.
- Reports unsafe conditions in the home.
- Practices effective infection control and adheres to infection control policies and procedures.
- Practices safe lifting and transferring of patients.
- Communicates clearly and positively with patients, families, team members and others.
- Displays a caring, patient’s first behavior.
- Keeps patients and/or family informed regarding visit time and schedule changes.
- Attends in-service programs to meet compliance requirements
- Maintains patient confidentiality in compliance with HIPAA
Basic Qualifications
- Must be a Certified Nursing Assistant (CNA) and/or possess current Home Health Aide registrar in the state of Indiana.
- Must have high school diploma or equivalent
- Must possess valid driver’s license and automobile insurance in the state of Indiana
- Must be able to provide own transportation to assignments.
- Ability to work independently
- Ability to work in and support a team environment.
