Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems throughout Indiana. As an extension of our partner hospital systems, we provide high-quality home health services to patients in the communities we serve.
We are excited to be expanding our services into the Tippecanoe County area and are looking for compassionate Registered Nurses to join our team. As a Home Health RN, you have the opportunity to provide personalized, one-on-one care while helping adult and geriatric patients remain safely and comfortably in their own homes. You will have the freedom and flexibility to manage your own schedule while still having the support of a collaborative clinical team.
Your days will be varied and rewarding as you use your nursing expertise to care for patients with a wide range of needs, educate families, and help patients achieve their goals at home.
As a Home Health Registered Nurse, you will:
Manage and evaluate individualized patient care plans.
Provide skilled nursing care to adult and geriatric patients in the home setting.
Provide wound care and chronic condition management
Administer and manage medications.
Educate patients and their families on medications, treatment plans, disease management, and ongoing care needs.
Monitor patient progress and changes in condition and communicate with the appropriate members of the care team.
Build trusting relationships with patients and families through compassionate, individualized care.
Coordinate care with physicians and other members of the interdisciplinary team.
Work independently in the field while remaining connected to a supportive clinical team.
Our Home Health RNs typically complete approximately 2 admission visits or 6 routine visits during a standard workday. This allows you to spend meaningful time with your patients while maintaining the flexibility and independence that makes home health nursing unique.
