Sunrise Services is seeking a Caregiver to join the 16-bed Enhanced Services Facility! This is a transitional facility supporting residents with a wide range of complex needs including physical disabilities, cognitive impairment, and or mental health support needs. Individuals with an active HCA or CNA are encouraged to apply!

Duties & Responsibilities:
Support and assistance is provided in areas including, but not limited to, the following:
A. Health and Safety
B. Power and Choice
C. Positive recognition by self and others
D. Integration into the physical and social life of the community
E. Developing relationships
F. Promoting competence and self-reliance
G. Maintaining records:
H. CARE assessments, Person Centered Service Plans and addendums, Behavioral Support Plan and other related documents if applicable:
Be able to describe the purpose of and have full and complete knowledge of the contents of the above plans. Follow these plans exactly as written.
I. Transportation In some cases may drive residents via agency owned or personal vehicle to / from medical / therapy appointments, grocery shopping or community activities.
