Founded in 1974, Swords to Plowshares is a community-based not-for-profit organization that provides counseling and case management, employment and training, housing, and legal assistance to veterans in the San Francisco Bay Area. We promote and protect the rights of veterans through advocacy, public education, and partnerships with local, state, and national entities. Job Title: Community Health Worker/Case Manager, Veteran Community Center (VCC)
Location: 1060 Howard Street, San Francisco, CA 94103
Schedule: Monday through Friday, 11 am – 7 pm
Reporting Type: In-person
Employment Type: Non-Exempt AND Full-Time
Salary Range: $35.33 per hour
Summary:
The Community Health Worker/Case Manager provides trauma-informed, person-centered support to individuals experiencing barriers related to housing instability, mental health, substance use, medical needs, and access to community resources. This position serves as a bridge between clients, healthcare providers, and community-based services by offering advocacy, care coordination, resource navigation, crisis intervention, and ongoing case management support.
The Community Health Worker/Case Manager works collaboratively with interdisciplinary teams to promote client wellness, stability, self-sufficiency, and engagement in services while fostering a safe and supportive environment.
Responsibilities:

· Coordinate referrals to housing, behavioral health, substance use treatment, benefits, employment, and other supportive services.

· Temporarily act as team lead in the absence of the Program Manager and Community Case Manager.

Employees who are regularly scheduled to work 20 hours or more per week are eligible for all benefits.

*Employees who work on-call, in a temporary position, or less than 20 hours per week regularly are only eligible for our Employee Assistance Program, paid lunch hour, and San Francisco sick time. Vacation and sick time are pro-rated based upon full-time equivalent percentage.
