Holistic Support Manager

At The Arc Baltimore, we support people with developmental disabilities in pursuing the lives they envision for themselves. As a Holistic Support Manager, you will bring together each person, their chosen team, Coordinators of Community Services, and other partners to coordinate services and keep the person’s goals at the center of the work. This role calls for strong communication, careful follow-through, and a commitment to protecting the rights of people we support.
What you’ll do
- Coordinate and facilitate person-centered planning with the person supported, their Coordinator of Community Services, and members of their self-identified team. Ensure plans meet applicable Developmental Disabilities Administration (DDA) and other regulatory requirements.
- Keep Person-Centered Plans current and accurate, reflecting each person’s preferences, goals, and services.
- Use progress notes, assessments, and other records to identify strengths and barriers, and work with the team to build on strengths and address barriers.
- Respond promptly to people supported, families, caregivers, colleagues, Coordinators of Community Services, and other partners. Connect people with the Management of Funds team or Benefits Counseling when needed.
- Train and communicate regularly with direct support professionals about person-centered and behavioral plans, progress, and changes in support needs.
- Protect the fundamental rights of people supported in accordance with COMAR 10.22.04 and help bring incident reports to a timely, satisfactory conclusion.
- Complete progress reports, iCM documentation, incident reports, and other required records on time.
- Coordinate required state and federal funding authorizations. Submit requests, secure approvals and signatures before services begin, and track authorizations to support timely access to services and uninterrupted reimbursement.
- Identify and resolve funding, eligibility, and redetermination issues before they disrupt services.
- Attend required meetings and training; participate in committees, work groups, fundraising, and other Arc Baltimore events.
- Perform other duties as assigned.
What you’ll bring
- A high school diploma or GED. A bachelor’s degree in social work, psychology, special education, or a related field is preferred; five years of experience in the disability field may be considered in place of a bachelor’s degree.
- At least one year of case management and supervisory experience in the disability field. Five years of experience as a Direct Support Professional may be substituted.
- Excellent written and verbal communication skills.
- Proficiency with Microsoft Office and other workplace technology.
Working conditions
Some evening and weekend work is required. You must have a valid Maryland driver’s license, an approved driving record, access to a personal vehicle for agency business—including transporting people supported—and a personal cell phone for availability during scheduled work hours. The role may require lifting up to 50 pounds and travel among Arc Baltimore locations; company travel reimbursement is offered. You must be able to work effectively as part of a team.
