Position Summary
The Outreach Specialist conducts outreach activities to newly referred and current PACE program clients, as well as disengaged clients, to market services, connect clients with resources, and re-engage clients.
Your Responsibilities Will Include
Participant Outreach & Engagement
- Conduct outreach activities to connect new, current, and disengaged participants with program services and resources.
- Conduct targeted re-engagement efforts to support participant participation and retention.
- Provide follow-up and support to encourage compliance with program requirements.
- Assist returning participants with successfully reconnecting to program services.
- Identify participant needs, barriers, and provide appropriate referrals and resources.
Communication, Support & Accessibility
- Utilize translation services, including Geneva Worldwide, to support participants with language barriers.
- Provide culturally appropriate communication and support to diverse participant populations.
- Identify and support reasonable accommodation needs in accordance with ADA requirements.
- Ensure participants receive clear information regarding program expectations, services, and available resources.
Community Outreach & Internal Collaboration
- Conduct community-based outreach activities to promote program awareness and participant engagement.
- Develop and maintain relationships with community partners and referral sources.
- Collaborate with Case Managers, Employment Specialists, Trainers, and internal departments to support participant needs.
- Assist with developing outreach strategies and improving participant engagement methods.
- Support the creation of referral pipelines between program departments.
Documentation & Program Compliance
- Maintain accurate and timely documentation of all outreach activities and participant interactions.
- Record outreach efforts, referrals, and follow-up activities in required databases.
- Process participant referrals according to program guidelines and procedures.
- Maintain confidentiality and comply with Fedcap, HRA, and program requirements.
- Complete assigned tasks and responsibilities in a timely and professional manner.
Administrative & Professional Responsibilities
- Perform other duties and special projects as assigned.
- Complete assigned tasks within established timelines and quality standards.
- Maintain professionalism, accountability, and flexibility in supporting program needs.
- Follow organizational policies, procedures, and program expectations.
You’re a Great Fit for This Role If You Have
Education & Experience
- High School Diploma or GED/TASC required.
- Minimum 1 year of full-time administrative support experience required.
- Bilingual in Spanish preferred.
Compensation
$45,000 per year/$21.64 per hour
