Are you ready to make a real difference in people's lives? Join the Harris Center for Mental Health and IDD as a Care Coordinator at our Emancipation Center. The Care Coordinator is responsible for clinical assessments, development of recovery plans in collaboration with the treatment team and provision of services based on consumer needs utilizing Trauma Informed and Patient Centered principles, and compliance with performance contract requirements. This role includes coordinating, linking and monitoring appropriate services, as well as assessing consumer progress towards recovery goals. Delivers interventions such as skills training, psychosocial rehabilitative services, case management, care coordination, and medication training/monitoring and supports. Care Coordinators are community-based positions which means the person must be able to travel to consumer homes or other field locations when providing services (as needed). Unit specific needs are determined by unit leadership including the portion of each day that may be allotted for clinic-based versus community-based service provision. Specific units may require staff to transport consumers; deliver medications; work evenings, weekends, holidays, and emergency closures; as well as be on-call evenings and weekends.
