Associate Program Director– Connect for Health Program:
The Institute for Family Health is seeking a full time master’s level social worker or equivalent to work as part of our interdisciplinary, patient centered team. The Connect for Health program provides health care and social services to unhoused, marginally housed and other at-risk persons at several locations in Manhattan – including two church-based programs and a center for LGBTQ youth. The Associate Program Director will work primarily at with these three program sites providing both clinical and case management services.
The Associate Program Director will provide services including crisis intervention, psychosocial assessments, suicide assessments and safety planning, supportive counseling, and coordination with medical and psychiatric providers. They will also provide and oversee medical case management services including outreach, referrals, Medicaid enrollment, assistance with other public benefits, specialty care and other services for uninsured and coordinate all aspects of care with the health care team. The Associate Program Director helps manage the daily workflow and supervises the team case manager.
- Provides direct social work services, which include case management, care navigation, crisis intervention, individual, family and group counseling in collaboration with other members of an interdisciplinary team and completion of relevant documents, psychosocial evaluations or service plans.
- Supervises program social workers and case managers/care navigators. Meets on a regular basis, individually or in groups to provide supervision.
- Supervises front desk officer/security at All Angels site
- Promotes interdisciplinary approach to patient care.
- Responsible for performance evaluations of social workers, case managers/care navigators, front desk officer.
- Develops and promotes new programs to meet previously unmet needs.
- Updates CFH program team re relevant issues related to psychosocial services
- Conducts and participates in quality assurance activities and promote high quality, comprehensive, effective patient care.
- Informs/updates psychosocial staff regarding relevant social policy issues.
- Represents department at site management and administration meetings as needed/appropriate;
- Responsible for task and clinical supervision for social work student at sites.
- Assists with the development, implementation and evaluation of department and site goals and objectives.
- Responsible for routine chart review of social workers and case managers;
- Prepares necessary statistical reports as requested;
- Ensure documentation by social service staff is in compliance with organization expectations.
- Oversees orientation of new social service staff.
- Maintain competency with regard to EPIC functionality to maximize usage and be aligned with agency and departmental workflows i.e.: Care teams, Treatment plans, Scheduling, Billing, Sliding fee assessments.
- Participates in and helps in the coordination of staff development programs.
- Collaborates with IFH Behavioral Health and Social Support Services Departments to ensure CFH program is up to date on relevant information, protocols, documentation, and resources.
- Organizes and implements education programs regarding timely and relevant social issues.
- Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient’s health problems.
- Assures social services compliance with legal, regulatory and professional standards, including article 28 and JCAHO.
Qualifications
- Fosters strong interdisciplinary relationships with medical providers, nursing staff, and other care providers, and community/collateral partners through case conferences, huddles, and warm-handoffs.
- Demonstrates flexibility in the acceptance and completion of work assignments.
- Participates in the department’s performance improvement activities.
- Maintains patient/employee confidentiality in the management of information.
- Observes the Health Care System’s compliance policies.
- Willing to supervise interns and attend required supervisor (SIFI) course.
- Participates in the development of other staff members.
- Meet regulatory, licensure and annual health assessment requirements.
- Identifies learning strengths and needs and utilizes learning resources
- Assists in the identification of needs of the program and partners with leadership in problem solving.
Education
- Bachelors of Science required.
- Master’s Degree in Social Work, Public Health, or related field preferred.
- NYS licensure that corresponds to Masters’ Degree, if applicable (LCSW, LMSW, or PsyD/PhD) required.
Work Experience and Skills
- Experience with program administration or management preferred
- Experience administering grant-based contracts preferred
- Experience using an electronic medical record or other electronic documentation systems preferred
- Two years experience in HIV, healthcare and/or social service settings required.
- Basic computer and internet navigational skills required.
- Computer literacy with Windows-based operating systems and MS Office applications (Word, Excel, Outlook, Powerpoint) required.
- Familiarity with electronic health records (EHR) technology preferred.
- Spanish preferred.
- French or French Creole preferred.
