    If you’re a Physical Therapist (PT) who wants to build meaningful relationships with your patients and see the long-term impact of your care—this is it.In this setting, you will work with Medicare Part B patients in Assisted Living and Independent Communities, allowing you to follow patient progress over time and focus on improving independence, mobility and quality of life.Unlike fast-paced short-stay environments, this role allows you to provide purposeful, patient-centered therapy, with consistency and clinical autonomy.     Responsibilities

    • Evaluate and treat residents in Assisted Living & Independent Living Communities
    • Develop personalized care plans focused on mobility, safety, and independence
    •  Deliver care through therapeutic exercise, functional mobility, balance activities, and manual techniques
    • Help patients improve strength, balance, and overall quality of life
    • Collaborate with care teams, staff, and caregivers to achieve strong patient outcomes
    • Document patient progress and adjust treatment plans as needed

    Qualifications:

    • Licensed Physical Therapist (PT) or license eligible in the state of practice
    • Graduate of an accredited Physical Therapy Program
    • Interest or experience in geriatric rehabilitation or senior living therapy
    • Passion for patient-centered care and improving quality of life for older adults
