Nurse Practitioner – Infusion Center (Part-Time)

The TwelveStone Infusion Centers LLC, manages the Infusion needs of patient’s and maintains the continuum of care, from intake, assessment, and treatment through the completion of their visit. Our primary focus is the safe and effective administration of biologic medications (e.g., Remicade, Tysabri, etc.), but we also offer a full range of infusion services including, but not limited to, antibiotics, hydration, vitamins, iron, and steroids. On occasion there is a need for sterile dressing changes and lab draws.

We are currently hiring a Part Time Nurse Practitioner for our infusion center located in Columbia, SC. This position is responsible for providing comprehensive care, including the assessment, diagnosis, treatment, management, education, health promotion and care coordination for patients and families with acute and chronic health needs in an outpatient setting. We welcome NP's from all medical backgrounds with a positive attitude, preferably outstanding IV skills, and an active license in the states of practice.

Responsibilities include:

• Provides patient and family centered IV infusion care. Facilitates consistent, coordinated care and clear communication among all members of the healthcare team. Incorporates the patient and family in care planning and treatment across settings. Serves as an advocate for patients and families in navigating the health care system.
• Communicates and collaborates effectively with physicians, health professionals, health related agencies, patients and families. Works effectively with multidisciplinary team members. Promotes mutual respect and trust working within a team model. Reviews, updates and recommends relevant medical, clinical and administrative policies,
• procedures and education & skills to help assure high quality patient care.
• Actively participates in quality initiatives. Maintains accurate and timely records and reports, participates in patient service and program reviews.
• Other duties as assigned.
