Position Summary
The Lead Chronic Care Physician provides direct patient care to individuals within the Department of Corrections while providing clinical leadership and oversight for the Chronic Care provider team. This position manages patients with complex medical conditions, supports quality improvement and quality assurance initiatives, provides clinical consultation and education to providers and staff, and assists with the development and implementation of treatment guidelines and care delivery models.
The Lead Chronic Care Physician also coordinates provider schedules and coverage, supports compliance with NCCHC standards, and collaborates with medical leadership to promote quality care, patient satisfaction, staff effectiveness, and efficient clinic operations.
Key Responsibilities
- Diagnose, manage, educate, and counsel patients on a broad range of healthcare issues, including complex medical conditions.
- Perform minor procedures, prescribe medications, and make referrals for specialty services as appropriate.
- Provide medical care during emergencies, including stabilization and life-support measures as necessary.
- Maintain accurate documentation of medical evaluations, procedures, treatments, referrals, consultations, and outcomes.
- Provide clinical training and technical consultation to physicians, advanced practice providers, and nursing staff.
- Participate in the development, implementation, and evaluation of clinical treatment guidelines and service delivery models.
- Support compliance with NCCHC standards and accreditation requirements.
- Monitor chronic care and referral logs and ensure appropriate follow-up and disposition.
- Supervise, schedule, and lead the Chronic Care provider team to support effective clinic operations and patient flow.
- Assist with chart reviews, case summaries, quality improvement activities, and clinical responses as needed.
- Coordinate provider schedules and coverage.
- Assist with identifying supply and equipment needs.
- Collaborate with medical leadership and staff to improve quality of care, productivity, and provider, staff, and patient satisfaction.
- Provide feedback to Medical Leadership regarding staffing and hiring needs.
- Assist with annual provider evaluations.
- Support orientation and onboarding of new providers and serve as a clinical resource.
- Perform other duties as assigned.
Minimum Qualifications
- Graduation from an accredited medical school.
- Valid license to practice medicine in Washington, DC.
- Current DEA registration and CDS certificate.
- Board certification in Internal Medicine.
- Experience leading physicians and Advanced Practice Providers (PAs/NPs) strongly preferred.
- Demonstrated ability to work effectively within a multidisciplinary healthcare team.
- Ability and willingness to work rotating schedules or shifts at DOC facilities, as needed.
- Proficiency with computers, office automation, and web-based tools.
- 5–10 years of experience in an ambulatory or inpatient healthcare setting preferred.
- Correctional healthcare experience preferred.
