Pharmacist
We are looking for an experienced Pharmacist at our location UPMC Cancer Center Zabok.
Primary Duties and Responsibilities:
The Pharmacist at UPMC Hillman Cancer Center Croatia will be responsible for ensuring the provision of cost-effective pharmacotherapy contributing to optimal patient outcomes, providing experiential education for present and future health care practitioners, and supporting drug-related research. They also may conduct health and wellness screenings, provide immunizations, oversee the medications given to patients, and provide advice on healthy lifestyles. These services will be provided in compliance with country and regional regulations as well as UPMC Hillman Cancer Center and center-specific policies.
Responsibilities
- Interpret and evaluate physician orders in order to ensure appropriate medication dosage forms and doses for outpatient chemotherapeutic treatment in accordance with the laws and regulations.
- Review, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medication orders.
- Facilitate compliance with cancer center directives by evaluating and overseeing the appropriate use of high-cost and highly toxic therapies in order to assure suitable control of drug budgets and reimbursement.
- Assist with the decision-making process in the selection of appropriate evidence-based prophylaxis, chemotherapeutic, and supportive care regimens based on the extent of disease, patient performance status and desired outcome for the treatment of a variety of malignant and nonmalignant conditions.
- Advise nursing staff in the administration risks of chemotherapies and immunotherapies.
- Assist in the develop medication-related treatment protocols in collaboration with the consultants.
- Adhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing consultative pharmaceutical care.
- Communicate with consultants and nurses to ensure prompt clarification of orders that are unclear, illegible, or incomplete.
- Provide drug information and/or educational programs to healthcare professionals and pharmacy students.
- Utilize all available technologies in the provision of consultative pharmaceutical care (e.g., medical records)
- Assist in the education of patients around side effects and potential toxicity of chemotherapeutic agents.
- Assist with education and counseling to patients regarding side effects, toxicity, and proper administration of medications.
- Identify and report suspected adverse drug reactions accurately and in a timely manner.
- Advise the clinical team in the identification and proper management of adverse drug events.
- Perform other duties as assigned.
Required qualifications:Education and Knowledge Requirements
- Degree in Pharmacy from an accredited school of pharmacy required.
- 3-6 months of hospital pharmacy experience is preferred.
- Previous oncology experience is preferred.
- Proficiency in reading, writing, and speaking English and Croatian is required.
Licenses and Certifications
- Appropriate country, region, and county licensure and certification may be required to meet minimum education and knowledge requirements.
