    About Us
A Specialty Path to Good Health

Upperline Health is the nation’s largest provider dedicated to lower extremity, wound and vascular care. Founded in 2017 with the ambitious goal of changing specialty care, Upperline Health delivers a more efficient path for patients to receive consistent and effective treatment for chronic illnesses.
    Triage is temporary.
    Treatment is transformative.
With over 130 podiatric medical and surgical clinics in 7 states, Upperline Health providers coordinate patients’ care among a team of specialists – podiatric physicians and surgeons, advanced practice providers, care navigators, pharmacists, dieticians, and social workers for integrated treatment that addresses patients’ immediate and long-term health needs.
We put patients at the center of value-based care.

    About the Endocrinologist Role

Upperline Health is seeking a well-trained and compassionate Endocrinologist (MD/DO) to join our growing specialty care team. This position provides a unique opportunity to practice within a multidisciplinary care model designed to proactively manage diabetes and endocrine disorders, with a strong focus on preventing complications such as lower-extremity wounds and amputations. This position is a clinic-based setting with the ability to provide both consultative and longitudinal care, supported by care coordination teams and advanced technologies.

As Upperline continues to expand its innovative approach to complex chronic care, this role offers long-term career growth, leadership development, and partnership opportunities.

    • Order, perform, and interpret relevant labs, imaging, and diagnostic tests.
    • Educate patients and families on disease progression, medication adherence, technology use (e.g., CGMs, insulin pumps), and lifestyle modification.
    • Document visits and treatment plans thoroughly and accurately in our electronic medical record system (EMR).
    • Serve as a key medical leader in interdisciplinary case reviews and care planning.
    • Support continuous quality improvement, clinical protocol development, and practice growth.
    • Maintain all appropriate licensure and certifications, including CME and board requirements

    Experience and Qualifications

    • MD or DO degree with Board Certification (or Eligibility) in Endocrinology, Diabetes & Metabolism
    • Active medical license or ability to obtain one in the state of practice
    • DEA registration or eligibility required
    • Passion for serving high-risk, underserved, or chronically ill populations
    • Excellent communication and interpersonal skills to foster meaningful patient and team relationships
    • Familiarity with or willingness to learn value-based care models and interdisciplinary collaboration
    • Experience with CGMs, insulin pumps, and diabetes technologies is preferred but not required
    • Recent fellowship graduates encouraged to apply
