Community Health Worker (CBH BIGGS Alzheimer’s dept)

We are looking for a CHW who is passionate about supporting families caring for individuals living with Alzheimer's disease and other dementias. The ideal candidate would have:

• Strong compassion and empathy
• Excellent communication and interpersonal skills
• Knowledge of community resources
• Familiarity with dementia care and caregiver support

Role Responsibilities:

• Communicating with caregivers and families of individuals with dementia
• Providing resources, coaching, encouragement, and active listening
• Conducting home safety assessments
• Making outbound phone calls and electronic communications
• Completing occasional home visits
