Key Responsibilities

The Reentry Housing Coordinator serves as a housing subject-matter expert within the Division of Reentry and Rehabilitation, responsible for establishing, navigating, and expanding sustainable housing pathways for individuals transitioning from state facilities or community supervision. Operating under general direction, this specialist builds external partnerships across local housing authorities, federal grant networks, private landlords, and non-profit service providers. This role bridges the gap between immediate emergency shelter needs and long-term community stability by integrating housing access with transportation, employment, healthcare, and wrap-around support systems.

Qualifications

Essential Duties & Responsibilities

• Housing Network & Community Partnership Development: Cultivate and maintain a robust network of housing providers, private landlords, emergency shelters, rapid re-housing initiatives, and supportive housing programs across Utah. Act as the Division's primary liaison to regional Continuums of Care (CoCs) and local housing authorities.

• Individual Navigation & Service Delivery: Evaluate individual housing barriers (e.g., criminal background checks, credit history, eviction records) and map tailored stabilization plans. Coordinate interconnected wrap-around services—including employment, transportation, vital identification documents, and life skills—to ensure long-term housing retention.

• Pre-Release Planning & Transition Coordination: Collaborate directly with facility discharge planners and probation/parole officers to initiate housing planning 90 days prior to release, facilitating direct, warm handoffs to community housing upon release.

• Cross-System Communication & Safety Alignment: Serve as the functional bridge between non-profit housing entities and probation/parole supervision staff. Ensure housing plans comply with court-ordered supervision conditions while maintaining client safety and program integrity.

• Program Evaluation & Contract Tracking: Track housing placement timelines, retention rates, and referral outcomes in the electronic case management database. Evaluate performance for state-funded housing contracts to ensure compliance with quality standards.

Qualifications & Competencies

• Housing Systems Knowledge: Deep understanding of HUD programs, Section 8, Housing First models, Rapid Re-Housing, Permanent Supportive Housing, and local Utah housing inventory.

• Holistic Reentry Integration: Understanding of how stable housing intersects with workforce development, Medicaid/healthcare navigation, substance use recovery, and public transportation infrastructure.

• Barrier Identification & Risk Awareness: Knowledge of evidence-based practices, trauma-informed care, risk-reduction strategies, and the unique societal barriers facing justice-involved populations.

• Stakeholder Engagement: Proven ability to negotiate, build trust, and maintain strong relationships with private market landlords, community non-profits, and government agencies.
