As a Patient Services Coordinator (PSC) in Hospice, you play a critical role behind the scenes - ensuring patients and families receive timely, coordinated care when it matters most.

This is a role for someone who thrives in organization, communication, and fast-paced environments - someone who can connect clinicians, patients, and schedules seamlessly to support exceptional end-of-life care.

If you’re detail-oriented, compassionate, and driven by purpose, this is where you belong.

    • Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination
    • Monitor schedules and follow up on missed, incomplete, or delayed visits
    • Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization
    • Partner with field teams and leadership to reduce missed visits and improve patient experience
    • Communicate effectively with patients, families, clinicians, and referral partners
    • Respond to changing patient needs and scheduling challenges with urgency and problem-solving
    • Maintain accurate documentation and workflow completion within the EMR system
    • Support quality initiatives, including participation in QAPI programs
    • Promote a culture of compassion and professionalism in every interaction

    What We’re Looking For

We’re seeking individuals who thrive in a fast-paced, team-oriented healthcare environment:

    • High school diploma or equivalent required
    • 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred)
    • Strong organizational and multitasking skills
    • Experience working with electronic medical records (EMR) or scheduling systems preferred
    • Excellent communication and interpersonal skills
    • Ability to prioritize, adapt, and problem-solve in real time

    What Sets You Apart

    • Experience coordinating schedules in a healthcare or service-based environment
    • Strong sense of urgency and accountability - takes ownership and follows through
    • Ability to manage multiple priorities without losing attention to detail
    • Calm, professional approach in high-pressure or emotionally sensitive situations
