Job Summary
The VNA RN Case Manager is responsible for providing skilled care and case management to patients/families in accordance with the State Nurse Practice Act and accepted professional standards of practice. The RN Case Manager provides services utilizing nursing theory and processes, communicates/documents observations and assessments, and promotes and maintains the health of individuals through direct care, teaching, counseling, and appropriate rehabilitative measures.Qualifications

Other Certificate/Diploma Nursing required or Bachelor's Degree Nursing preferred

Experience
Minimum of one (1) year of experience in a home care/hospice setting or equivalent experience required Hospice experience preferred
Knowledge, Skills and Abilities
- Graduate of an approved, accredited school of professional nursing required.
- Demonstrated knowledge of home health/hospice regulatory and reimbursement requirements required.
- Demonstrated excellent verbal and written communication skills and strong interpersonal skills required.
- Demonstrated strong organizational and time management skills required.
- Good driving record and reliable transportation required.
