Geriatric Care Manager

Monday - Friday 9am -4pm

In this role, you’ll be a trusted partner to members and their families, helping older adults navigate the complexities of healthcare and long-term care services while supporting their desire to remain independent in the home and community of their choice. You’ll help members understand their options, coordinate services and resources, and navigate health changes, care transitions, and evolving needs.

You’ll use your experience, judgment, and problem-solving skills to assess situations, anticipate needs, and determine appropriate next steps. You’ll collaborate closely with the Thrive at Home team, healthcare professionals, and community providers to ensure members receive coordinated and responsive support.

• Demonstrated experience coordinating services and navigating complex healthcare and community-based systems.
• Experience communicating and connecting with older adults, adult children, caregivers, healthcare professionals, and community providers.
• Working knowledge of the continuum of care, including hospitals, skilled nursing facilities, rehabilitation, home health, non-medical home care, assisted living, memory care, palliative care, hospice, and community-based services.
• Experience with care transitions and discharge planning strongly preferred.
• Familiarity with long-term care insurance, aging resources, and community supports is highly desirable.
• Demonstrated ability to independently manage a caseload, prioritize competing needs, follow through on commitments, and recognize when a situation requires escalation.
• Strong assessment, problem-solving, documentation, communication, and relationship-building skills.
