Pharmacist
To support the interprofessional team in delivering wholistic, culturally safe, client centered care by providing expert pharmaceutical knowledge and evidence informed medication management. The role will continue to evolve in response to changes in professional scope of practice, organizational priorities, and client care needs.
As a member of an interprofessional team, the Pharmacist provides clinical pharmacy services that enhance the quality, safety, and effectiveness of medication use across WNHAC programs and services. The Pharmacist works collaboratively with clients, healthcare providers, and community partners to support optimal health outcomes through comprehensive medication management and pharmaceutical care.
NOTE: Traditional practices involving burning sacred medicines including tobacco, sweetgrass, sage and cedar and other cultural protocols occur regularly within our work setting.
PRIMARY DUTIES / RESPONSIBILITIES:
- Provide comprehensive clinical pharmacy services, including medication assessments, medication reconciliation,
- Medication reviews, and therapeutic recommendations to optimize medication use and client outcomes.
- Collaborate with physicians, nurse practitioners, nurses, and other members of the interprofessional team to optimize medication therapy. Prescribe, adapt, renew, and monitor medications within professional scope of practice and organizational policies to support continuity of care and timely access to treatment.
- Educate clients, families, caregivers, and healthcare providers regarding the safe, effective, and appropriate use of medications to support informed decision-making and medication adherence.
- Promote medication safety and quality improvement by identifying and resolving medication-related issues and contributing to organizational policies, procedures, and best practices.
- Provide pharmacist-authorized clinical services, including assessment and treatment of minor ailments, immunization administration, and other authorized clinical services.
- Collaborate with community pharmacies, hospitals, and other healthcare providers to facilitate medication access, support continuity of care, and coordinate medication-related communication and follow-up.
- Maintain accurate, timely, and complete clinical documentation in the electronic medical record and other required systems in accordance with organizational policies and professional standards.
- Actively participate in organizational teams, committees, and quality improvement initiatives, as required.
QUALIFICATIONS:
- Bachelor of Science in Pharmacy (BScPhm) or Doctor of Pharmacy (PharmD) from an accredited institution, and current registration in good standing with the Ontario College of Pharmacists;
- Demonstrated knowledge of medication management, pharmacotherapy, and evidence-informed clinical practice, with the ability to apply sound professional judgment in a primary care setting.
- Knowledge of current Ontario pharmacy legislation, standards of practice, and applicable prescribing and injection authority. knowledge of community-based primary health care, health promotion, chronic disease management, and community resources.
- Demonstrated oral and written communication skills; excellent interpersonal skills to deal effectively with a wide variety of people;
- Demonstrated ability to work in a collaborative, de-colonial, trauma-informed, and client-centered approach and to support WNHAC values in meaningful ways;
- Deep understanding and experience of Indigenous culture, values, and perspectives and relevance in a workplace; ability to speak and understand Anishinaabemowin a definite asset;
- Experience working in an Indigenous organization; understanding and/or willingness to learn of Indigenous culture and values;
- Proof of immunization in compliance with policy requirements is mandatory;
- Valid drivers’ license, own vehicle and willingness and ability to travel required;
- Criminal records check and current drivers’ abstract required;
- Commitment to ongoing training and professional development relevant to job requirements; and positive attitude and capacity to act as a healthy lifestyle role model.
Salary: $100,778 - $131,492 with excellent benefits, including HOOPP pension plan. Salary is based on education and experience.
