Community Agency for Senior Citizens (CASC) is a nonprofit organization dedicated to helping older adults remain safely and independently in their homes and communities. Through case management and supportive services, CASC works to identify client needs, connect individuals with resources, and improve quality of life for seniors across Staten Island.
The Care Manager provides case management services to adults age 60 and older by identifying client needs and service preferences. The role works with clients and, when appropriate, their families to develop, implement, and oversee individualized care plans based on mutually agreed-upon goals. Care Managers connect clients with community services, provide benefits and entitlement counseling, advocate for client needs, and support access to appropriate services.
